LATE REGISTRATION INSTRUCTIONS

Email the following information to
addisonrecclub@ameritech.net : player name, indicate boy or
girl, grade in school, and parent name and telephone numbers.

print out
registration form. Fill out the form and mail it with a
check for full registration fee plus the fundraiser buyout to
179 N. Catalpa St., Addison, IL 60101.

The registration form must be filled
out completely with no blank spaces and no unanswered questions.

The registration must be paid in
full and this includes the fundraiser buyout.

Circling "yes" for interest in
travel team does not bind the player to play but you will be
notified about the date, time, and place of the tryout for
the travel team.

The travel teams play after the
in-house season and does not conflict with other sports programs
such as Indian Trail basketball team, Rec Club in-house teams,
volleyball teams.
